Examples: Xanax (alprazolam), Ativan (lorazepam), Valium (diazepam), Klonopin (clonazepam)

Please be aware that our practice does not prescribe benzodiazepines. If you are currently taking one prescribed by another physician, we are happy to discuss your care but will not continue this class of medication.

Before you submit

ADHD evaluations: If you are seeking an evaluation for ADHD, please be aware that a complete diagnostic evaluation typically requires two to three sessions. Medication will not be prescribed prior to the completion of the evaluation.

Intake paperwork: Once your appointment is scheduled, you will receive intake forms through the patient portal. This paperwork must be completed at least 24 hours before your appointment or your appointment will be rescheduled.
